摘要:
目的:观察EB病毒感染患儿免疫学、肝功能指标及其相关性。方法:选取EB病毒感染患儿83例为研究组,并以EB病毒感染阴性者50例为对照组。比较两组患儿免疫学指标[自然杀伤(NK)细胞、T细胞、B细胞、辅助/诱导T细胞、调节/抑制T细胞、CD4+/CD8+比值]及肝功能指标[丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、乳酸脱氢酶(LDH)]。分析患儿免疫学指标与肝功能指标的相关性,以及两者与EB病毒感染的相关性。采用多元Logistic回归分析导致EB病毒感染患儿出现不良预后的独立危险因素。结果:研究组患儿NK细胞、B细胞、CD4+/CD8+比值低于对照组,T细胞、辅助/诱导T细胞、调节/抑制T细胞、ALT、AST、LDH水平高于对照组(P<0.05)。患儿NK细胞、B细胞、辅助/诱导T细胞、CD4+/CD8+比值与ALT、AST水平呈负相关,T细胞、调节/抑制T细胞与ALT、AST、LDH水平呈正相关(P<0.05)。患儿NK细胞、B细胞、辅助/诱导T细胞、CD4+/CD8+比值与EB病毒感染呈负相关,T细胞、调节/抑制T细胞、ALT、AST、LDH水平与EB病毒感染呈正相关(P<0.05)。多元Logistic回归分析显示,热峰>40 ℃、免疫功能紊乱、肝功能异常、多浆膜腔积液是导致患儿出现不良预后的危险因素(P<0.05)。结论:EB病毒感染患儿存在免疫功能紊乱及肝功能异常的现象,其免疫学指标与肝功能指标具有相关性,且肝功能损伤及免疫功能紊乱是导致患儿出现不良预后的独立危险因素。
Abstract:
Objective:To observe the immunology,liver function indexes and their correlation in children with EB virus infection.Methods:83 children with EB virus infection were selected as study group,and 50 negative EB virus infection children were enrolled as control group.The immunological indexes such as NK cells,T cells,B cells,helper/induced T cells,regulatory/inhibitory T cells,CD4+/CD8+ ratio and liver function indexes such as ALT,AST,LDH were compared between two groups.The correlation between immunological indexes and liver function indexes,as well as their correlation with EB virus infection were analyzed.Multivariate Logistic regression analysis was used to analyze the independent risk factors of adverse prognosis in children with EB virus infection.Results:NK cells,B cells,CD4+/CD8+ ratio in study group were lower than those in control group,while the levels of T cells,helper/induced T cells,regulatory/inhibitory T cells,ALT,AST and LDH in study group were higher than those in control group(P<0.05).NK cells,B cells,helper/induced T cells,CD4+/CD8+ ratio were negatively correlated with ALT and AST levels,while T cells,regulatory/inhibitory T cells were positively correlated with ALT,AST and LDH levels(P<0.05).NK cells,B cells,helper/inducer T cells,CD4+/CD8+ ratio were negatively correlated with EB virus infection,while T cells,regulatory/inhibitory T cells,ALT,AST,LDH levels were positively correlated with EB virus infection(P<0.05).Multivariate Logistic regression analysis showed that heat peak more than 40 ℃,immune function disorder,liver function abnormality and polyserositis were the risk factors of poor prognosis(P<0.05).Conclusion:There are immune function disorder and abnormal liver function in children with EB virus infection,and the immunological indexes are correlated with liver function indexes,and liver function damage and immune function disorder are independent risk factors leading to poor prognosis.
