摘要:
目的:探讨肥胖人群肌肉分布特点及其与内脏脂肪型肥胖的关系。方法:采用InBody720多频生物电阻抗分析仪对2318例成人进行人体成分分析。按体重指数(BMI)不同将受试者分为正常组、超重组及肥胖组,分析各组人群肌肉量特点及其与内脏脂肪的关系,并确定肌肉量指标与内脏脂肪型肥胖(以内脏脂肪面积>100 cm2作为的诊断标准)的最佳切点。结果:除双上肢肌肉量/体重之外,其他数据在三组间比较均有统计学差异(P<0.05)。将双下肢肌肉量/体重与肥胖相关指标进行分析显示,校正了年龄和性别后,内脏脂肪含量与腰臀比、体脂率呈正相关(r=0.720、0.843,均P<0.05),与双上肢肌肉量/体重、双下肢肌肉量/体重均呈负相关(r=-0.557、-0.880,均P<0.05)。双下肢肌肉量/体重预测内脏脂肪型肥胖(内脏脂肪面积>100 cm2)的ROC曲线下面积为0.922。以最大约登指数确定切点,当下肢肌肉量/体重为0.2115时,对内脏脂肪型肥胖诊断的敏感性为85.6%,特异性为82.8%。结论:双下肢肌肉量/体重可作为评价内脏脂肪型肥胖的潜在有效指标。
Abstract:
Objective:To investigate the association between distribution characteristics of muscle and visceral obesity in obese population.Methods:The human body composition of 2318 adults was analyzed by inbody720 multi frequency bioelectrical impedance analyzer.According to different body mass index(BMI),the subjects were divided into normal group,overweight group and obesity group.The characteristics of muscle mass and the relationship between visceral fat and muscle mass index were analyzed,and the optimal cut-off point between muscle mass index and visceral fat obesity(visceral fat area >100 cm2)was determined.Results:Except for the muscle mass of both upper limbs/body weight,other data were statistically different among the three groups(P<0.05).After adjusting for age and gender,visceral fat content was positively correlated with waist hip ratio and body fat rate(r=0.720、0.843,all P<0.05),and negatively correlated with upper limb muscle mass / body weight,lower limb muscle mass/body weight(r=-0.557、-0.880,all P<0.05).The area under the ROC curve for predicting visceral fat obesity(visceral fat area >100 cm2)was 0.922.When the muscle mass of lower limbs/body weight was 0.2115,the sensitivity and specificity of diagnosing visceral adiposity were 85.6% and 82.8%,respectively.Conclusion:Muscle mass of both lower limbs/body weight can be used as a potential effective index to evaluate visceral fat obesity.
