摘要:
目的:探讨计算机辅助设计与制造(CAD/CAM)牙种植导板在上前牙列缺失即刻种植修复中的应用价值。方法:将需即刻种植修复的上前牙列缺失患者96例随机分为研究组与对照组,各48例,对照组给予常规牙种植修复术治疗,研究组在对照组的基础上联合应用CAD/CAM牙种植导板辅助治疗。比较两组术后植入体的空间位置偏离值,两组术前术后的咀嚼效率、言语功能评分、龈沟出血评分、菌斑指数与软垢指数,观察两组术后种植牙体的美学效果(外形、色泽)、骨吸收值。结果:研究组的各空间位置偏离值(头部、根部、深度、角度)均明显小于对照组(P<0.05); 两组术后咀嚼效率、言语功能评分均明显高于术前(P<0.05),龈沟出血评分、菌斑指数与软垢指数、骨吸收值均明显低于术前(P<0.05),术后研究组咀嚼效率、言语功能评分均明显高于对照组(P<0.05),龈沟出血评分、菌斑指数与软垢指数均明显低于对照组(P<0.05); 两组术后种植牙体的外形与色泽比较均无统计学差异(P>0.05)。结论:CAD/CAM牙种植导板能提高上前牙列缺失患者即刻种植修复术的种植精准度,提高咀嚼功能、言语功能,减轻骨吸收,改善牙周环境。
Abstract:
Objective:To explore the application value of computer-aided design and manufacturing(CAD/CAM)dental implant guide template in immediate implant restoration of anterior dentition missing.Methods:96 patients with anterior dentition missing who needed immediate implant restoration were randomly divided into study group and control group,48 cases in each group.The control group was treated with routine dental implant restoration,while study group was additionally treated with CAD/CAM dental implant guide template.The spatial position deviation values of implants after surgery,chewing efficiency,scores of speech function and sulcus bleeding,plaque index and debris index before and after surgery were recorded and compared between the two groups.The aesthetic effects of implants(appearance,color)and bone resorption value after surgery were observed and compared between the two groups.Results:The spatial position deviation values(head,root,depth,angle)in study group were significantly lower than those in control group(P<0.05).After surgery,chewing efficiency and scores of speech function in both groups were significantly increased(P<0.05),while sulcus bleeding score,plaque index,debris index and bone resorption value were significantly decreased(P<0.05).After surgery,chewing efficiency and scores of speech function in study group were significantly higher than those in control group(P<0.05),while sulcus bleeding score,plaque index and debris index were significantly lower than those in control group(P<0.05).After surgery,there were no significant differences in the appearance and color of the dental implants between the two groups(P>0.05).Conclusion:CAD/CAM dental implant guide template can improve the implant accuracy of immediate implant restoration in patients with anterior dentition missing,improve chewing function and speech function,reduce bone resorption,and improve periodontal environment.
