摘要:
目的:探讨乙肝肝炎患者乙肝病毒血清学标志物(HBV-M)模式、前S2抗原(Pre-S2)与HBV-DNA定量检测的关系。方法:选择180例乙型病毒性肝炎患者,近期均未接受相关抗病毒治疗。采用双抗夹心酶联免疫吸附法(ELISA)检测乙肝病毒血清标志物水平和前S2抗原,采用荧光定量聚合酶链反应(qRT-PCR)检测血清HBV-DNA定量,对HBV-M检出模式、Pre-S2阳性率情况与HBV-DNA定量检测结果进行分析。结果:共检出14种HBV-M模式,其中大三阳、小三阳和其他HBV-M模式分别占比25.56%、46.11%、28.33%,不同HBV-M模式HBV-DNA、Pre-S2阳性率比较均有统计学差异(P<0.05); 107例HBV-DNA阳性患者中,HBsAg、Pre-S2阳性检出率为93.46%、95.33%,明显高于HBeAg阳性率 47.66%%(P<0.05),且随着HBV-DNA载量水平升高,HBeAg阳性率也明显升高(P<0.05); 不同HBV-DNA载量患者HBsAg、Pre-S2阳性率均明显高于HBeAg阳性率(P<0.05)。结论:HBV-M、HBV-DNA定量和Pre-S2检测结果紧密相关,联合检测能为HBV感染及病情评估提供充分依据。
Abstract:
Objective:To explore the relationship between serum markers(HBV-M)pattern,Pre-S2 antigen(Pre-S2)and quantitative detection of HBV-DNA in patients with hepatitis B hepatitis.Methods:180 patients with Hepatitis B were selected.None of them received antiviral therapy recently.Serum HBV markers and Pre-S2 antigen were detected by ELISA.Serum HBV-DNA was detected by fluorescence quantitative polymerase chain reaction(qRT-PCR).HBV-M detection pattern,HBV-DNA positive rate and load,Pre-S2 positive rate were analyzed.Results:A total of 14 HBV-M patterns were detected,of which the big three positives,small three positives and other HBV-M patterns accounted for 25.56%,46.11%,28.33%,respectively.The positive rates of HBV-DNA and Pre-S2 in different HBV-M patterns were statistically different(P<0.05).among the 107 HBV-DNA positive patients,the positive detection rates of HBsAg and Pre-S2 were 93.46% and 95.33%,which were significantly higher than the HBeAg positive rate of 47.66%%(P<0.05),and with the increase of HBV-DNA load level,the positive rate of HBeAg also increased significantly(P<0.05).The positive rates of HBsAg and Pre-S2 in patients with different HBV-DNA load were significantly higher than the positive rate of HBeAg(P<0.05).Conclusion:The results of HBV-M,HBV-DNA and PRE-S2 are closely related.The combined detection can provide sufficient evidence for HBV infection and disease evaluation.
