摘要:
目的:检测妊娠糖尿病(GDM)患者血清维生素D、白细胞介素-17水平及胰岛素抵抗指数(HOMA-IR),并探讨其相关性。方法:选取48例妊娠糖尿病孕妇(GDM组),另选取同期年龄匹配的正常孕妇作为对照组。收集两组一般资料,检测外周静脉血三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)、空腹血糖(FPG)、空腹胰岛素(FINS)、白细胞介素-17(IL-17)、25羟维生素D3[25(OH)D3]等生化指标,计算HOMA-IR; 采用Logistic回归模型分析GDM的独立相关因素; 采用Pearson相关性分析法分析GDM患者血清25(OH)D3、IL-17及HOMA-IR的相关性。结果:两组年龄、BMI、孕周、TC、TG、HDL-C、LDL-C比较无统计学差异,GDM组FPG、FINS、IL-17、HOMA-IR水平高于对照组,25(OH)D3水平显著低于对照组; GDM组维生素缺乏程度高于对照组; 多因素Logistic回归结果提示25(OH)D3水平降低(OR:0.731,95%CI:0.557~0.957),IL-17水平升高(OR:1.287,95%CI:1.056~1.568)是GDM的独立危险因素; Pearson相关分析结果显示血清25(OH)D3与HOMA-IR呈负相关(r=-0.615,P<0.05),血清IL-17与HOMA-IR呈正相关(r=0.538,P<0.05),血清25(OH)D3与IL-17呈负相关(r=-0.456,P<0.05)。结论:GDM患者相比正常孕妇维生素D缺乏更为明显,血清维生素D水平下降、IL-17水平升高与胰岛素抵抗密切相关,可能增加GDM患病风险。
Abstract:
Objective:To detect serum vitamin D,interleukin-17(IL-17)levels and insulin resistance index(HOMA-IR)in patients with gestational diabetes mellitus(GDM),and to explore their correlation.Methods:48 pregnant women with GDM(GDM group)were enrolled.And the normal pregnant women with age-matched during the same period were selected as control group.The general data in both gruops were collected.The biochemical indexes in peripheral venous blood such as triglyceride(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),fasting plasma glucose(FPG),fasting insulin(FINS),IL-17 and 25(OH)D3 were detected.The homeostasis model assessment of insulin resistance(HOMA-IR)was calculated.The independent related factors of GDM were analyzed by Logistic regression model.The correlation among serum 25(OH)D3,IL-17 and HOMA-IR in GDM patients was analyzed by Pearson correlation analysis.Results:There were no significant differences in age,BMI,gestational week,TC,TG,HDL-C and LDL-C between the two groups.The levels of FPG,FINS,IL-17 and HOMA-IR in GDM group were significantly higher than those in control group,while level of 25(OH)D3was significantly lower than that in control group.The severity of vitamin deficiency in GDM group was significantly higher than that in control group.The results of multivariate Logistic regression indicated that decreased level of 25(OH)D3(OR=0.731,95%CI:0.557-0.957)and increased level of IL-17(OR=1.287,95%CI:1.056-1.568)were independent risk factors of GDM.The results of Pearson correlation analysis showed that serum 25(OH)D3 was negatively correlated with HOMA-IR(r=-0.615,P<0.05),serum IL-17 was positively correlated with HOMA-IR(r=0.538,P<0.05),and serum 25(OH)D3 was negatively correlated with IL-17(r=-0.456,P<0.05).Conclusion:Compared with normal pregnant women,vitamin D deficiency is more significant in GDM patients.The decrease of serum vitamin D level and the increase of IL-17 level are closely related to insulin resistance,which may increase GDM risk.
