摘要:
目的:探讨H型高血压患者同型半胱氨酸血症(Hcy)水平及不同动态血压模式与心脏结构、功能变化间的关系。方法:选择276例原发性高血压患者,依照患者血Hcy水平将患者分为对照组与研究组,其中研究组纳入H型高血压患者(Hcy≥10 μmol/L,141例); 对照组纳入非H型高血压患者(Hcy<10 μmol/L,135例),比较两组患者一般资料、生化指标、动态血压变化等数据,同时对患者进行心脏彩色多普勒超声检测,收集并计算相关数据。结果:研究组Hcy水平显著高于对照组(P<0.05); 研究组患者24 h平均收缩压、24 h平均脉压、白天平均收缩压、白天平均脉压、室间隔厚度、左心室后壁厚度、左心室舒张末期内径、左心室质量、左心室质量指数显著高于对照组(P<0.05); 研究组患者非杓型血压发生率显著高于对照组(P<0.05); 多元线性回归分析显示:左心室质量指数与Hcy正相关(β=0.182,P<0.05)。结论:Hcy水平的变化会显著影响患者血压变异性(BPV),Hcy与BVP的变化是导致患者左心室重构及心脏功能改变的主要影响因素。
Abstract:
Objective:To investigate the relationship between the level of homocysteine(Hcy),different ambulatory blood pressure patterns and the changes of cardiac structure and function in patients with H-type hypertension.Methods:276 patients with essential hypertension were selected and divided into control group and study group according to Hcy level.The study group included 141 patients with H-type hypertension(Hcy≥10 μmol/L).The control group included 135 patients with non-H-type hypertension(Hcy<10 μmol/L).The general data,biochemical indexes and ambulatory blood pressure changes of the two groups were compared.At the same time,the patients were detected by color Doppler ultrasound,and the relevant data were collected and calculated.Results:Hcy level in the study group was significantly higher than that in the control group(P<0.05).24-hour mean systolic pressure,24-hour mean pulse pressure,daytime mean systolic pressure,daytime mean pulse pressure,ventricular septal thickness,left ventricular posterior wall thickness,left ventricular end diastolic diameter,left ventricular mass,left ventricular mass index were significantly higher than those in the control group(P<0.05).The incidence of non dipper blood pressure in the study group was significantly higher than that in the control group(P<0.05)Multiple linear regression analysis showed that left ventricular mass index was positively correlated with Hcy(β=0.182,P<0.05).Conclusion:The changes of Hcy level can significantly affect the blood pressure variability(BPV).The changes of Hcy and BVP are the main influencing factors of left ventricular remodeling and cardiac function change.
