摘要:
目的:探讨高血压前期人群血压昼夜节律与中性粒细胞淋巴细胞比值的关系。方法:选取受试者244例进行研究。所有受试者均检测身高、体重、血糖、血脂、血钠、血钾及24 h动态血压,根据24 h动态血压结果将入选者分为杓型组和非杓型组,比较两组受试者一般资料并分析中性粒细胞淋巴细胞比值(NLR)与血压昼夜节律的相关性。结果:①与杓型组受试者相比,非杓型组受试者体重指数、甘油三酯、低密度脂蛋白胆固醇、NLR及C反应蛋白(CRP)均显著升高,差异有统计学意义(P<0.01)。②与杓型组高血压前期患者相比,非杓型组高血压前期患者24 h舒张压平均值、24 h平均动脉压值、夜间收缩压平均值、夜间舒张压平均值差异有统计学意义(P<0.01); 夜间收缩压下降率、夜间舒张压下降率均明显降低,比较差异有统计学意义(均P<0.01)。③夜间收缩压下降率与年龄、体重指数(BMI)、甘油三酯、NLR呈负相关(P<0.05),夜间舒张压下降率与年龄、BMI、NLR、甘油三酯、低密度脂蛋白胆固醇均呈负相关(P<0.05); 在校正了BMI、甘油三酯、低密度脂蛋白胆固醇等危险因素后,NLR仍为高血压前期患者夜间血压下降率的独立危险因素(P<0.05)。结论:高血压前期人群非杓型组中性粒细胞淋巴细胞比值升高,是血压昼夜节律改变的相关因素。
