摘要:
目的:探讨应用二维斑点追踪技术评价不同体质量指数(BMI)2型糖尿病患者左心房功能。方法:选取初次诊断2型糖尿病患者110例,根据患者BMI不同分为体质量正常组(BMI≤24 kg/m2,30例)、超重组(24 kg/m22,40例)和肥胖组(BMI≥28 kg/m2,40例)。另外选取30例健康志愿者作为正常对照组。应用二维斑点追踪技术获取心尖四腔切面、二腔切面、三腔切面左心房各节段纵向应变值,分别计算左心室收缩期左心房整体峰值纵向应变(LAS-s)、舒张早期左心房整体纵向应变(LAS-e)、舒张晚期左心房整体纵向应变(LAS-a)。比较四组间各参数的差异。结果:与正常对照组比较,体质量正常组、超重组、肥胖组的LAS-s、LAS-e和LAS-a水平均减低,组间比较差异均有统计学意义(均P<0.05)。超重组LAS-a大于体质量正常组与肥胖组(均P<0.05)。结论:随着BMI增加,2型糖尿病患者左心房心肌功能受损趋严重。超重和肥胖是2型糖尿病患者左心房功能受损的主要因素。二维斑点追踪技术能够早期发现左心房心肌结构和功能的改变情况。
Abstract:
Objective:To analyze left atrial function in type 2 diabetic with different body mass index(BMI)by two-dimensional speckle tracking imaging(2D-STI).Methods:110 type 2 diabetes patients were selected and divided into three group based on the different BMI:normal weight group(BMI≤24 kg/m2,30 subjects),overweight group(24 kg/m22,40 subjects),obesity group(BMI≥28 kg/m2,40 subjects).Another 30 healthy volunteers were selected as the normal control group.The strain of the left atrial segments on the four-chamber section,two-chamber section and three-chamber section of the apex of the heart were calculated by two-dimensional speckle tracking imaging(2D-STI).LA peak ventricular systolic strain(LAS-s),LA peak early-diastolic strain(LAS-e)and LA peak atrial systolic strain(LAS-a)in longitudinal directions of all the subjects were measured by 2D-STI.The differences of parameters among the four groups were compared.Results:Compared with the normal control group,the levels of LAS-s,LAS-e and LAS-a in the normal weight group,the overweight group and the obesity group were all decreased,and the differences between the groups were significant(all P<0.05).The LAS-a of the overweight group was higher than that of the normal weight group and the obesity group(all P<0.05).Conclusion:With the increase of BMI,the myocardial function of left atrium in type 2 diabetes patients tends to be seriously damaged.Overweight and obesity are the main factors of left atrial dysfunction in patients with type 2 diabetes.2D-STI can detect the structural and functional changes of the left atrium in the early stage.
