摘要:
目的:比较左束支起搏与右室间隔起搏的电学参数、近期临床效果及安全性。方法:选取因缓慢性心律失常行永久性心脏起搏器植入术患者50例,随机分成两组,心室电极分别植入左束支区域与右室间隔部。术后1周及3个月行起搏器程控,测定两组患者起搏电学参数、QRS波宽度及不良事件等。结果:短期随访两组患者起搏阈值、感知无统计学差异(P>0.05); 术后阻抗均较术中有所下降(P<0.05),QRS波时限左束支区域起搏者较短(P<0.01); 术后左心功能指标均无明显变化(P>0.05); 均未发生严重不良事件。结论:左束支区域起搏对缓慢性心律失常患者短期疗效及安全性良好,起搏参数稳定。
Abstract:
Objective:To compare the pacing parameters,short-term clinical efficacy and safety of left bundle branch pacing(LBBP)and right ventricular septal pacing(RVSP)in the treatment of patients with bradyarrhythmia. Methods:Fifty patients with bradyarrhythmia undergoing permanent pacemaker implantation were selected and divided into two groups. The ventricular electrodes were implanted into the left bundle branch and the right ventricular septum. Pacemaker program control test was carried out one week and three months after operation. The pacing parameters,QRS wave width were measured and adverse events were recorded. Results:There was no significant difference in pacing threshold and sensing parameter between the two groups(P>0.05).The postoperative impedance was lower than that in the operation(P<0.05). The QRS duration in the left bundle branch was reduced(P<0.01).No complications were found. There was no significant change in left heart function index after operation(P>0.05). Conclusion:Left bundle branch pacing has good short-term efficacy and safety in patients with bradyarrhythmia,and the pacing parameters are stable.
