摘要:
目的:探究中性粒细胞与肾综合征出血热病情危重化的关系。方法:收集肾综合征出血热早期患者126例,将其分为非危重型(85例)与危重型(41例)两组; 采用非参数检验和t检验比较中性粒细胞及其他实验室检验指标在两组之间的差异,并用二元Logistic 回归与受试者工作特征曲线探索它们在疾病早期预警方面的价值。结果:两组之间NEUT最高值病日、NEUT、NEUT%、PLT、PLR、BUN、Cr、Ca2+、APTT、PT、住院天数的差异有统计学意义(Z=-2.328、4.84、t=-2.05、Z=-5.175、-4.715、2.235、2.519、-4.005、5.136、2.783、5.090,均P<0.05),二元Logistic 回归筛选出NEUT、Cr、APTT可以作为疾病危重的独立影响因子(P=0.025、0.027、0.001),三者联合预测的曲线下面积为0.856(P<0.001),敏感度为0.795,特异性为0.817。结论:中性粒细胞数目与HFRS病情危重化密切相关,可以作为预测病情危重的独立影响因子,与Cr、APTT联合有更好的预测效力。
