摘要:
目的:分析2型糖尿病患者致动脉粥样硬化各危险因素与冠脉狭窄严重程度的相关性。 方法:收集行冠脉造影检查明确诊断为冠心病合并2型糖尿病患者188例。采用双变量线性回归分析方法,分析空腹血糖、糖尿病病程、糖化血红蛋白(HbA1c)、餐后2 h血糖、血脂等指标与冠脉狭窄程度之间的相关性; 采用多元线性回归分析比较各因素对冠脉狭窄严重程度的作用大小。冠脉狭窄程度采用定量Gensini评分的方法进行记录。结果:Gensini评分与相关临床指标的双变量线性相关分析显示,年龄(r=0.185,P=0.009)、糖尿病病程(r=0.362,P<0.001)、糖化血红蛋白(r=0.272,P<0.001)与Gensini评分呈正相关。高密度脂蛋白胆固醇(r=-0.237,P=0.001)与Gensini评分呈负相关。Gensini评分与其他指标无明显相关性。多元线性回归分析显示,糖尿病病程、糖化血红蛋白的升高和Gensini评分的升高存在剂量-反应关系,在校正冠心病相关危险因素后,糖尿病病程、糖化血红蛋白均进入回归方程,Gensini评分随着糖尿病病程、糖化血红蛋白的升高而升高。 结论:糖尿病病程和糖化血红蛋白是影响冠心病合并2型糖尿病患者冠脉狭窄的重要因素。
Abstract:
Objective:To investigate the correlation between atherogenic risk factors and severity of coronary stenosis in patients with type 2 diabetes mellitus.Methods:188 patients with coronary heart disease diagnosed by coronary angiography and combined with type 2 diabetes mellitus were collected.The correlation between fasting blood glucose,course of diabetes mellitus,HbA1c,2 hours postprandial blood glucose,blood lipid and the degree of coronary stenosis was analyzed by bivariate linear regression analysis.Multiple linear regression analysis was used to compare the effects of various factors on the severity of coronary stenosis.The degree of coronary stenosis was recorded by quantitative Gensini score.Results:The bivariate linear regression analysis showed that the age(r=0.185,P=0.009),duration of diabetes(r=0.362,P<0.001),HbA1c(r=0.272,P<0.001)were positively correlated with Gensini score,and high density lipoprotein cholesterol(r=-0.237,P=0.001)was negatively correlated with Gensini score,and there was no significant correlation between Gensini score and other indexes.Multiple linear regression analysis showed that there was a dose-response relationship between the diabetic duration,the increase of glycosylated hemoglobin and the increase of Gensini score; after correcting the risk factors related to coronary heart disease,the diabetic duration and glycosylated hemoglobin entered the regression equation,and the Gensini score increased with the increase of diabetic duration and glycosylated hemoglobin.Conclusion:The course of diabetes mellitus and glycosylated hemoglobin are important factors affecting coronary stenosis in patients with coronary heart disease and type 2 diabetes mellitus.
