摘要:
目的:固醇调节元件结合蛋白(SREBPs)是一种重要的转录因子,对调控哺乳动物的脂质稳态至关重要。本研究的目标是探讨冠心病(CAD)合并代谢综合征(MS)患者外周血白细胞SREBPs表达水平与脂代谢指标的相关性。方法:收取健康对照者159例,CAD患者70例,CAD合并MS患者73例。空腹抽取静脉血测定各组临床生化指标,实时荧光定量PCR检测外周血白细胞中SREBPs(SREBP-1,SREBP-2)的mRNA表达水平。结果:与健康对照组相比,CAD组与CAD合并MS组外周血白细胞SREBP-1 mRNA表达水平显著降低。在所有样本中,SREBP-1与总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、载脂蛋白A1(Apo-A1)、胱抑素-C(Cys-C)呈正相关,与高敏C反应蛋白质(hs-CRP)、肌酸激酶-MB(CK-MB)、同型半胱氨酸(HCY)呈负相关; 在疾病样本中,SREBP-1与HDL-C、GSP呈正相关,与HCY、hs-CRP呈负相关。Logistic回归分析显示,SREBP-1可能是CAD合并MS的潜在风险因素[优势比(OR)= 0.346,95%可信区间(CI)=(0.191,0.625),P <0.001]。ROC曲线分析显示,单独用SREBP-1预测CAD合并MS严重程度指标时,灵敏度较高但特异度较低; 联合HDL-C或Apo-A1预测诊断后,特异度明显升高但灵敏度较低。结论:SREBP-1可能成为诊断CAD合并MS的潜在生物学标志物,与脂代谢指标联合有更大的评估和诊断价值。
Abstract:
Objective:To investigate the correlation between the expression levels of SREBPs in peripheral blood leukocytes and lipid metabolism in patients with coronary heart disease(CAD)complicated with metabolic syndrome(MS). Methods:159 healthy controls(control group),70 CAD patients(CAD group)and 73 CAD patients with MS(CAD-MS group)were collected. Fasting venous blood was taken to determine the clinical biochemical indicators,and real-time fluorescence quantitative PCR was used to detect the mRNA expression levels of SREBPs(SREBP-1,SREBP-2)in peripheral blood leukocytes. Results:Compared with the control group,the expression levels of SREBP-1 mRNA in the peripheral blood of the CAD group and the CAD-MS group were significantly reduced. In all samples,SREBP-1 was positively correlated with TC,HDL-C,Apo-A1 and CysC,but negatively correlated with hs-CRP,CK-MB and HCY. In disease samples,SREBP-1 was positively correlated with HDL-C and GSP,but negatively correlated with hs-CRP and HCY. Logistic regression analysis showed that SREBP-1 might be a potential risk factor for CAD complicated with MS(OR=0.346,95%CI 0.191 to 0.625,P<0.001). ROC curve analysis showed that SREBP-1 alone had higher sensitivity but lower specificity in predicting the severity index of CAD combined with MS,while HDL-C or Apo-A1 combined with SREBP-1 had higher specificity but lower sensitivity.Conclusion:SREBP-1 may become a potential biomarker for the diagnosis of CAD complicated with MS,and the combined diag-nosis with HDL-C and Apo-A1 has greater evaluation and diagnostic value,but a larger sample size is needed to further verify the results of this experiment.
