摘要:
目的:通过检测脑出血患者发病早期神经元特异性烯醇化酶(NSE)的含量,研究NSE的含量和脑出血患者病情及早期预后的关系。方法:80例脑出血患者为观察组,75例同期健康体检志愿者为对照组; 通过电化学发光法测定80例观察组发病12 h内静脉血NSE含量、对照组清晨空腹静脉血NSE含量。应用美国国立健康研究所制定的急性脑卒中评定表(NIHSS)评估脑出血患者入院时的神经缺损情况,应用格拉斯哥预后评分(GOS)评估患者治疗3~6个月后的恢复情况。结果:①观察组脑出血患者发病12 h内血清NSE浓度明显高于对照组,差异有统计学意义(P<0.01)。②脑出血患者出血量和发病12 h内血清NSE浓度相关,出血量越大,NSE浓度越高,差异有统计学意义(P<0.01)。脑出血患者出血部位和血清NSE浓度无相关性(P>0.05)。③脑出血患者神经功能缺损越严重,血清NSE浓度越高,差异有统计学意义(P<0.01)。④预后不良组患者发病12 h内血清NSE浓度明显高于预后良好组,差异有统计学意义(P<0.01)。结论:NSE检测对脑出血患者病情严重程度、早期预后判断有一定的临床参考价值。
Abstract:
Objective:To study the relationship between the content of neuron-specific enolase(NSE)and the condition and early prognosis of patients with cerebral hemorrhage by detecting the content of NSE in the early stage of cerebral hemorrhage.Methods:80 patients with cerebral hemorrhage were taken as the observation group,and 75 healthy volunteers during the same period were taken as the control group.The NSE content in the venous blood of the observation group within 12 hours after onset and the fasting venous blood NSE content of the control group in the morning were determined by electrochemiluminescence.The Acute Stroke Assessment Form Developed by National Institutes(NIHSS)was used to assess neurological deficits in patients with cerebral hemorrhage at admission,and the Glasgow Prognostic Score(GOS)was used to assess patients' recovery after 3 to 6 months of treatment.Results:The serum NSE concentration in the observation group was significantly higher than that in the control group within 12 hours after onset of cerebral hemorrhage,and the difference was statistically significant(P<0.01).The amount of bleeding in patients with cerebral hemorrhage was correlated with the serum NSE concentration within 12 hours after onset.The greater the amount of bleeding,the higher the NSE concentration,and the difference was statistically significant.There was no correlation between the bleeding site and serum NSE concentration in patients with cerebral hemorrhage.The more severe the neurological deficit in patients with cerebral hemorrhage,the higher the serum NSE concentration,and the difference was statistically significant(P<0.01).The serum NSE concentration in patients with poor prognosis was significantly higher than that in patients with good prognosis within 12 hours after onset,and the difference was statistically significant(P<0.01).Conclusion:NSE detection has high clinical value in the diagnosis of severity and early prognosis of patients with cerebral hemorrhage.
