摘要:
目的:探讨噻托溴铵粉联合糖皮质激素治疗慢性阻塞性肺疾病患者的疗效及对患者QOL评分的影响。方法: 选取慢阻肺患者72例,采用单盲试验将患者分为两组,对照组使用糖皮质激素丙酸氟替卡松吸入气雾剂治疗,研究组使用噻托溴铵粉雾剂联合丙酸氟替卡松吸入气雾剂治疗。比较两组患者运动耐力、肺功能改善情况及全身炎症反应、免疫功能、生活质量情况。结果:治疗后,研究组运动耐力明显高于对照组(P<0.05),FEV1/FVC、FEV1均高于对照组(P<0.05); 临床疗效优于对照组(P<0.05); 血清CRP、TNF-α、IL-6水平均低于对照组(P<0.05); 免疫功能指标均高于对照组(P<0.05); 生活质量优于对照组(P<0.05)。结论:慢阻肺患者使用噻托溴铵粉联合糖皮质激素治疗,运动耐力有明显提升,肺功能改善良好,临床疗效显著,且患者全身炎症反应明显减轻,免疫功能明显提高,生活质量明显改善。
Abstract:
in the treatment of COPD and its effect on QOL score of patients.Methods:72 patients with COPD were selected and divided into two groups by a single-blind test.The control group was treated with fluticasone propionate inhaled aerosol,and the study group was treated with tiotropium bromide combined with fluticasone propionate inhaled aerosol.The exercise endurance,lung function improvement,systemic inflammatory response,immune function and quality of life were compared between the two groups.Results:After treatment,the exercise endurance in the study group was significantly higher than that in the control group(P<0.05),and FEV1/FVC and FEV1 were both higher than those in the control group(P<0.05).The clinical efficacy of the study group was better than that of the control group(P<0.05).Serum CRP,TNF-α and IL-6 levels of the study group were all lower than those of the control group(P<0.05).The immune function indexes of the study group were higher than those of the control group(P<0.05).The quality of life of the study group was better than that of the control group(P<0.05).Conclusion:The treatment of tiotropium bromide combined with glucocorticoid for patients with COPD can obviouslyimprove exercise endurance,lung function,and clinical efficacy,reduce systemic inflammatory response,improve immune function and patients' quality of life.
