摘要:
目的:探讨非布司他治疗早期糖尿病肾病伴高尿酸血症患者的临床疗效及安全性。方法:早期糖尿病肾病伴高尿酸血症患者82例,随机分为试验组42例(其中有2例脱落),对照组40例,分别接受非布司他及苯溴马隆治疗,疗程均为6个月。比较两组患者治疗期间血尿酸(UA)、血肌酐(Scr)、血尿素氮(BUN)、肾小球滤过率(eGFR)和24 h尿蛋白定量(24 h ACR)等指标。结果:治疗后3个月、6个月,试验组患者的UA水平均显著低于对照组(P<0.05)。治疗后6个月试验组患者的肾功能均较治疗前及治疗3个月均有显著改善(P<0.05),24 h ACR差异无统计学意义(P>0.05),但与对照组比较差异有统计学意义(P<0.05)。试验组不良反应的总发生率显著低于对照组(P<0.05)。结论:非布司他治疗早期糖尿病肾病伴高尿酸血症患者能够有效地降低血尿酸水平,改善患者肾功能,安全性高。
Abstract:
Objective:To investigate the clinical efficacy and safety of febuxostat in the treatment of patients with early diabetic nephropathy and hyperuricemia.Methods:Eighty-two patients with early diabetic nephropathy and hyperuricemia were randomly divided into the experimental group(n=42)and the control group(n=40).They were treated with febuxostat and benzbromarone for 6 months,respectively.Blood uric acid(UA),serum creatinine(Scr),blood urea nitrogen(BUN),glomerular filtration rate(eGFR)and 24 h urine protein quantitation(24 h ACR)were compared between the two groups.Results:At 3 months and 6 months after treatment,the UA levels in the experimental group were significantly lower than those in the control group(P<0.05).At 6 months after treatment,the renal function of the experimental group was significantly improved compared with that before treatment and at 3 months after treatment(P<0.05).There was no significant difference in 24 h ACR(P>0.05),but it was different from the control group with statistical significance(P<0.05).The overall incidence rate of adverse reactions in the experimental group was significantly lower than that in the control group(P<0.05).Conclusion:Febuxostat can effectively reduce blood uric acid level and improve renal function in patients with early diabetic nephropathy and hyperuricemia,and it is highly safe.
