摘 要 目的:探究心外膜脂肪(EAT)体积及血浆可溶性血栓调节蛋白(sTM)表达水平与冠心病病变程度间相关性。方法:选择109例入院接受诊治的冠心病患者(研究组)和同期进行健康体检的45例同龄段健康志愿者(对照组)为研究对象。螺旋CT对两组受试者EAT进行测定;依据冠脉造影(CAG)图像计算研究组患者Gensini评分,并依据评分将其分为低分者、中分者和高分者;双抗体夹心酶联免疫吸附法测定两组sTM含量。分析EAT体积及血浆sTM含量与冠心病病变程度相关性。结果:与对照组比,研究组患者EAT体积明显增大,血浆sTM含量均明显增加(P<005);冠心病患者EAT体积和sTM含量曲线下面积分别为0.911(95%CI:0.862~0.960)和0.895(95%CI:0.838~0.953)。对照组和研究组不同Gensini积分患者EAT体积和血浆sTM间差异均具有统计学意义(P<005),研究组中低分者、中分者和高分者EAT体积和血浆sTM含量均明显大于对照组受试者,中分者和高分者显著大于低分者,高分者显著大于中分者(P<005)。Pearson相关系数法分析显示,冠心病患者EAT体积及血浆sTM含量均与Gensini积分呈现显著正相关(r=0.512、0.436,均P<0.01)。结论:冠心病患者EAT体积及血浆sTM含量与冠脉Gensini积分间呈显著正相关,患者EAT体积越大,血浆sTM含量越高,冠脉病变程度越严重。
Abstract:
Abstract Objective:To investigate the relationship between the volume of epithelial adipose tissue(EAT) ,the cotent of plasma soluble thrombomodulin(sTM) and the severity of coronary heart disease. Methods:A total of 109 patients with coronary heart disease admitted to the hospital(study group) and 45 healthy volunteers with the same physical examination(control group) were selected as subjects. Spiral CT was used to quantify EAT in both groups. The Gensini scores of the study group were calculated according to the coronary angiography(CAG) image,and were classified into low scorer,middle scorer and high scorer according to the score;the sTM content of the two groups were determined by double antibody sandwich enzymelinked immunosorbent assay. The correlation between EAT volume and plasma sTM content and severity of coronary heart disease was analyzed. Results:Compared with the control group,the EAT volume and the plasma sTM content of the study group were significantly increased(P<005). The area under the curve of the EAT volume and sTM content in patients with coronary heart disease was 0.911(95%CI:0.862 to 0.960) and 0.895(95%CI:0.838 to 0.953),respectively. There were significant differences in EAT volume and plasma sTM between the control group and the study group with different Gensini scores(P<005),the EAT volume and plasma sTM content of the cases with the low,middle and high scores in the study group were significantly greater than those in the control group;the middle scorer and the high scorer were significantly higher than the low scorer,and the high scorer was significantly larger than the middle scorer(P<005). Pearson correlation coefficient analysis showed that EAT volume and plasma sTM content in patients with coronary heart disease were significantly positively correlated with Gensini score(r=0.512,0.436,both P<001). Concluion:There is a significant positive correlation between EAT volume,plasma sTM content and coronary Gensini score in coronary heart disease patients. The higher the EAT volume,the higher the plasma sTM content,the more serious the coronary lesions.
