摘 要 目的:探讨FloTrac/Vigileo监测下妇科腔镜手术不同角度头低脚高位血流动力学及灌注指数(PI)的变化。方法:将62例妇科腹腔镜手术患者随机分为A组(n=20)、B组(n=21)、C组(n=21),分别倾斜10°、20°、30°的头低脚高位。利用FloTrac/Vigileo 系统监测患者气腹前(T1)、气腹后1 min(T2)、改变体位时(T3)、气腹结束后1 min(T4)及气腹结束后5 min恢复体位时(T5)的平均动脉压(MAP)、中心静脉压(CVP)、心输出量(CO)、每搏量(SV)等血流动力学变化,利用血氧监测仪监测末梢灌注指数(TPI),并记录手术相关指标及术后并发症。结果:与T1时刻比较,各组T2~T3时刻HR、MAP均明显升高(P<005),C组T3时刻CO、SV均较T2时刻明显升高(P<005)。与T1时刻比较,各组T2~T3时刻TPI均下降(P<005),C组T3时刻TPI明显低于T2时刻,且低于A组、B组(P<005)。与A组、B组比较,C组CO、SV在T2~T3时刻明显升高(P<005)。随着倾斜角度的增加,气腹时间、手术时间随之缩短(P<005)。结论:FloTrac/Vigileo系统可实时监测妇科腹腔镜手术患者的血流动力学,而头低脚高位20°下行妇科腹腔镜手术是相对理想的倾斜角度,既能保证良好的手术视野,还能维持血流动力学和TPI稳定,缩短气腹和手术时间。
Abstract:
Abstract Objective:To explore the effects of diferent Trendelenburg angles on hemodynamics and perfusion index(PI) monitored by FloTrac/Vigileo system in patients undergoing gynecological laparoscopic surgery Methods:62 patients with gynecological laparoscopic surgery were randomly divided into group A (n=20),group B (n=21),group C (n=21),with tilt angles of 10°,20°,30°respectively FloTrac/Vigileo system was used to monitor hemodynamic changes including mean arterial pressure (MAP),central venous pressure (CVP),cardiac output (CO),and stroke volume (SV) at different time points during the operation,and blood oxygen monitor was used to monitor peripheral perfusion index (TPI),and relevant surgical indicators and postoperative complications were recorded Results:Compared with T1,HR and MAP were significantly increased at T2~T3 in each group (P<005),while CO and SV were significantly increased at T3 in group C compared with that at T2(P<005) Compared with T1,TPI in each group was decreased significantly at T2~T3 (P<005),and TPI in group C at T3 was significantly lower than that at T2,and lower than that of group A and group B (P<005) Compared with group A and group B,CO and SV in group C were increased significantly at T2~T3 (P<005) With the increase of tilt angle,the duration of pneumoperitoneum and surgery were shortened significantly(P<005) Conclusion:The FloTrac/Vigileo system can realtime monitor the gynecologic laparoscopic surgery in patients with hemodynamics,and gynecological laparoscopic surgery in the position of 20° is relatively ideal,which can ensure good operation field of vision,also can maintain hemodynamics and TPI stability,shorten the pneumoperitoneum and operation time
