摘 要 〖HT5K〗目的：观察新型包皮环切器结合改良镇痛方法在小儿包皮环切术中的临床应用效果。方法：选取行包皮环切术的小儿患者330例，将其随机分为三组。A组110例应用新型包皮环切器手术，采用改良镇痛方法（利多卡因乳膏于术前半小时阴茎涂抹表面麻醉，分别于术后半小时、术后第2天、换药前半小时用双氯芬酸钠栓剂50mg纳肛）；B组110例采用套扎器包皮环切，用传统镇痛方式（利多卡因阴茎根部阻滞麻醉，于术后半小时、术后第2天、换药前半小时口服双氯芬酸钠缓释片）。C组110例采用传统包皮环切术、传统镇痛方式（利多卡因阴茎根部阻滞麻醉，于术后半小时、术后第2天、换药前半小时口服双氯芬酸钠缓释片）。对三组患者围手术期相关指标进行比较分析。结果：所有手术均顺利完成，A组患者手术时间（3.42±0.88）min，术中出血量（2.45±0.52）ml，术中疼痛评分（0.05±0.01）分，术后6h疼痛评分（0.28±0.63）分 ，换药时疼痛评分（0.45±0.83）分， 伤口愈合时间（6.25±2.13）d ，外观满意度（98.6%），术后并发症（2.7%），各项观察指标均优于B、C两组，差异具有统计学意义（P＜0.05）。结论：新型包皮环切器结合改良镇痛方法在小儿包皮环切术及围手术期具有出血少、用时短、全程无痛、外观满意、恢复快、并发症少、患者易接受等优点，是治疗小儿包皮过长或包茎较好的方式。
Abstract:
Abstract 〖WT5BZ〗〖HT5”SS〗Objective：To observe the clinical application effect of new circumcision anastomat together with new analgesic method in pediatric circumcision. Methods：By selecting 330 from 580 cases of children receiving circumcision operation，and then randomly divide them into three groups labeled respectively as A，B and C. As for the 110 cases in Group A，the new circumcision anastomat together with the new analgesic method were adopted (the compound lidocaine cream was used to the penis for anaesthesia half an hour before the operation，and the diclofenac sodium suppository 50mg anus was respectively used half an hour，two days after the operation and half an hour before every dressing change)；As for the 110 cases in Group B，the ligation device circumcision together with traditional analgesic method are adopted (lidocaine penile root block anesthesia was used，also taking diclofenac sodium sustained release tablets orally half an hour，two days after the operation and half an hour before every dressing change respectively)；As for the 110 cases in Group C，the traditional circumcision together with traditional analgesic method are adopted (lidocaine penile root block anesthesia is used，also taking diclofenac sodium sustained release tablets orally half an hour，two days after the operation and half an hour before every dressing change respectively). Then，the relating indicators of the three groups during the periooperative period are compared and analyzed. Results：All the operations had been conducted smoothly. The specific indicators of Group A include：the operation time was （3.42±0.88）min，the intraoperative blood loss was （2.45±0.52）ml，the pain scores intraoperatively was （0.05±0.01），the pain scores 6 hours postoperatively was （0.28±0.63），the pain scores in dressing change was （0.45±0.83），the wound healing time was （6.25±2.13）d ，the appearance satisfaction is 98.6%，and the postoperative complications is 2.7%. In summary，all indicators of Group A were superior to that of Group B and Group C（P＜0.05）and the significant difference bears statistical significance. Conclusion：Many advantages have appeared in the application of the new circumcision anastomat together with the new analgesic method in pediatric circumcision during perioperative〖LM〗 period ，such as little blood loss，short operation time，painless whole course，high appearance satisfaction，quick operation recovery，less complications，agreeable to the patients etc，which is the comparatively better method at present for treating children’s redundant prepuce or phimosis，being worthy of promotion and application. 
