摘 要 目的：探究Orem自理模式联合连续被动活动（CPM）锻炼仪对股骨头置换患者术后髋关节功能的影响。方法：选择股骨头置换术的86例患者为研究对象，按照随机数字表法将其均分为实验组与对照组，每组各43例患者，对照组患者术后在常规康复训练的基础上增加Orem自理模式，实验组患者在对照组基础上加用CPM锻炼仪进行治疗，对比两组患者术前、术后7 d、术后21 d、术后60 d髋关节Harris评分，对比术前、术后60 d两组患者髋关节主动活动范围，对比两组患者术后并发症发生率、拄拐行走及脱拐行走时间。结果：①术前两组患者Harris评分对比差异不具有统计学意义（P＞0.05），术后7 d、21 d、60 d实验组患者Harris评分高于对照组（P＜0.05）；②术前两组患者髋关节主动活动范围对比差异不具有统计学意义（P＞0.05），术后实验组患者髋关节伸展、外展、内外旋角度均大于对照组（P＜0.05）；③术后实验组并发症发生率较低，拄拐行走时间及脱拐行走时间较短（P＜0.05）。结论：Orem自理模式联合CPM锻炼仪能够加快股骨头置换患者术后髋关节的功能恢复，同时改善其关节活动度，降低术后并发症发生率。
Abstract:
Abstract Objective:To explore the effect of Orem self-care model combined with continuous passive motion (CPM) exercise instrument on hip joint function after femoral head replacement.Methods:86 patients who underwent femoral head replacement in our hospital from January 2017 to January 2018 were divided into experimental group and control group according to the method of random number table. Each group had 43 patients. The patients in the control group were given Orem self-care model on the basis of routine rehabilitation training. The experimental group was given control group. The Harris score of hip joint before operation，7 days after operation，21 days after operation and 60 days after operation were compared between the two groups. The active range of motion of hip joint was compared between the two groups before operation and 60 days after operation.Results:①There was no significant difference in Harris score between the two groups before operation (P＞0.05). The Harris score of the experimental group was higher than that of the control group on the 7th，21th and 60th day after operation (P＜0.05);②There was no significant difference in active range of motion of the hip joint between the two groups before operation (P＞0.05). The hip joint extension，abduction，internal and external rotation angle of the experimental group were higher than those of the control group (P＜0.05);③The incidence of complications in the experimental group was lower，the walking time of crutches and the walking time of detachment were shorter (P＜0.05).Conclusion: Orem self-care model combined with CPM exercise instrument can accelerate the functional recovery of hip joint after femoral head replacement，improve its joint mobility and reduce the incidence of postoperative complications.
