摘 要 〖HT5K〗目的：探讨内镜逆行胰胆管造影术（ERCP）后出血的危险因素。方法：收集行ERCP术后并发出血的患者（出血组）43例，按1∶2的比例匹配在年龄、性别相近的ERCP术后未出血的患者86例作为对照组；分析比较两组患者在原发疾病、术中操作步骤等因素，选择单因素分析中P<0.05的因素作为潜在危险因素，进一步行多因素分析。结果：单因素分析结果显示：胆总管结石、括约肌预切开、括约肌大切开、球囊取石术、鼻胆引流管因素在出血组与对照组中均有统计学差异。将上述因素进行逻辑回归多因素分析显示：括约肌大切开（OR 3.245，95%CI 1.210～8.699）、括约肌预切开（OR 3.957，95%CI 1.395～11.223）、球囊取石 （OR 3.492，95%CI 1.371～8.898）为ERCP术后出血独立的风险因素。结论：括约肌大切开、括约肌预切开、球囊取石可能增加ERCP术后出血风险。
Abstract:
Abstract Objective：Post-Endoscopic Retrograde Cholangiopancreatography (ERCP）bleeding is one of the severe complications . and the risk factors for this condition have not yet been clearly evaluated . The current study aimed to investigate the risk factors of post-ERCP bleeding. Methods：The hemorrhage group consists of 43 patients who experienced post-ERCP bleeding whereas the control group has 86 patients matched for age and gender who have not experienced post-ERCP bleeding. We compared protopathy (e.g. Choledocholithiasis), technical (e.g.Long-incision of sphincter) factors between these two groups. Results：There were significant difference between the hemorrhage and the control group in choledocholithiasis, pre-cut of sphincter, long-incision of sphincter, balloon catheter stone extraction, endoscopic nasobilary. In logistic regression: pre-cut of sphincter(OR3.957, 95%CI1.395~11.223), long-incision of sphincter (OR 3.245, 95%CI 1.210~8.699), balloon catheterextraction (OR3.492, 95%CI1.371~8.898) were independent risk factors of post-EST bleeding. Conclusion：The current study illuminated that long-incision of sphincter, pre-cut of sphincter, balloon catheterextraction raise the likelihood of post-ERCP bleeding, therefore doctors and nurses should identify those patients with high risks and do things to prevent such.
