摘 要 目的: 研究了脾多肽注射液对脓毒症患者血清炎症因子及免疫功能的影响。方法: 选取重症监护室进行治疗的脓毒症患者30例,按照随机数字法分为对照组和脾多肽组,各15例。对照组采用常规治疗方法,脾多肽组在对照组的基础上使用脾多肽注射液进行静脉滴注。比较两组患者治疗前后的急性生理与慢性健康评分(APACHEⅡ)、白细胞计数(WBC)、心率以及相关脏器功能衰谒评分(SOFA)的变化。比较两组患者治疗前后血清炎症因子的变化,包括白介素10(IL10)、白介素6(IL6)、肿瘤坏死因子α(TNFα)和C反应蛋白(CRP)。比较两组患者治疗前后T淋巴细胞亚群的变化,包括CD4+、CD8+和CD4+/ CD8+。结果: 脾多肽组的住院时间显著低于对照组(P<0.05)。与治疗前相比,治疗后对照组和脾多肽组的APACHEⅡ评分、SOFA评分、WBC和心率均显著降低(P<0.05)。治疗后,与对照组相比,脾多肽组的APACHEⅡ评分、SOFA评分、WBC和心率均显著较低(P<0.05)。与治疗前相比,治疗后对照组和脾多肽组IL6、TNFα和CRP均显著较低(P<0.05),对照组和脾多肽组IL10显著增高(P<0.05)。治疗后,脾多肽组IL10、IL6、TNFα和CRP水平显著低于对照组(P<0.05),脾多肽组IL10水平显著高于对照组(P<0.05)。与治疗前相比,治疗后对照组和脾多肽组CD4+显著较高(P<0.05),对照组和脾多肽组CD8+和CD4+/ CD8+的变化与治疗前相比,差异无统计学意义(P>0.05)。治疗后,脾多肽组CD4+水平显著高于对照组(P<0.05),脾多肽组和对照组在CD8+和CD4+/ CD8+相比,差异无统计学意义(P<0.05)。结论:脾多肽注射液辅助治疗脓毒症,能够改善临床症状,降低住院时间和促炎因子水平以及提高患者的免疫功能。
Abstract:
Abstract Objective:To study the effects of spleen peptide injection on serum inflammatory factors and immune function in patients with sepsis. Methods:30 patients with sepsis were randomly divided into control group and spleen polypeptide group, 15 cases each. The control group was treated with conventional treatment, and the spleen polypeptide group was intravenously infused with spleen polypeptide injection on the basis of the control group. The changes of APACHE II, WBC, heart rate and SOFA before and after treatment were compared between the two groups. The changes of serum inflammatory factors before and after treatment were compared between the two groups, including IL10, IL6, TNFα and CRP. Changes in T lymphocyte subsets before and after treatment were compared between the two groups, including CD4+, CD8+, and CD4+/CD8+. Results:The hospital stay in the spleen polypeptide group was significantly lower than that in the control group (P<0.05). The APACHE II score, SOFA score, WBC and heart rate of the control group and the spleen polypeptide group were significantly lower than those before treatment (P<0.05). After treatment, the APACHE II score, SOFA score, WBC and heart rate were significantly lower in the spleen polypeptide group than in the control group (P<0.05). Compared with before treatment, IL6, TNFα and CRP in the control group and spleen polypeptide group were significantly lower (P<0.05), and IL10 in the control group and spleen polypeptide group was significantly increased (P<0.05). 〖LM〗After treatment, the levels of IL10, IL6, TNFα and CRP in the spleen polypeptide group were significantly lower than those in the control group (P<0.05), and the IL10 level in the spleen polypeptide group was significantly higher than that in the control group (P<0.05). Compared with before treatment, the CD4+ of the control group and the spleen polypeptide group was significantly higher (P<0.05), and the changes of CD8+ and CD4+/CD8+ in the control group and the spleen polypeptide group were not significantly different from those before treatment (P>0.05). After treatment, the CD4+ level in the spleen polypeptide group was significantly higher than that in the control group (P<0.05). There was no significant difference between the spleen polypeptide group and the control group in CD8+ and CD4+/CD8+ (P<0.05). Conclusion:Spleen polypeptide injection for the treatment of sepsis can improve clinical symptoms, reduce hospitalization time, reduce the level of proinflammatory factors and improve the immune function of patients, which is worthy of popularization in clinical practice.
