摘 要 目的:研究腹腔镜子宫(LH)和开腹子宫切除术(AH)对机体免疫功能的影响。方法:检测腹腔镜子宫(LH)30例和开腹子宫切除术(AH)30例患者术前24 h、术后48h的血清免疫球蛋白IgG、IgA、IgM,补体 C3、C4和 C反应蛋白水平进行对照研究。结果:术前LH组和AH组免疫球蛋白及补体浓度相比无统计学差异(P>0.05)。LH组手术前、后免疫球蛋白及补体均无显著变化(均P>0.05),AH组免疫球蛋白IgG、补体C3术后48h下降程度显著低于术前(P<0.05)。术前LH组和AH组C反应蛋白浓度相比无统计学差异(P>0.05)。虽然LH组和AH组术后48hC反应蛋白含量均明显高于术前(P<0.05),但是,AH组术后48h的血清C反应蛋白水平却显著高于LH组(P<0.05)。经比较两组术后临床指标,LH组术中出血少、术后最高体温低、术后住院日短。结论:LH创伤较轻,引起机体免疫功能变化小,是真正意义上的微创手术。
Abstract:
Abstract Objective: To study the effect of laparoscopic hysterectomy (LH) and open hysterectomy (AH) on immune function. Methods: the serum immunoglobulin IgG, IgA, IgM, complement C3, C4 and creactive protein levels of 30 patients with laparoscopic hysterectomy (LH) and 30 patients with open hysterectomy (AH) were measured 24 hours before surgery and 48 hours after surgery. Results: there was no difference in immunoglobulin and complement concentrations between LH group and AH group before operation (P>0.05). There were no significant changes in immunoglobulin and complement before and after operation in the LH group (all P were>0.05), and the decreased degree of immunoglobulin IgG and complement C3 in the AH group was significantly lower than that before operation (P<0.05). There was no difference in C reactive protein concentrations between the LH group and the AH group before surgery (P>0.05). Although the content of creactive protein was significantly higher in both the LH group and the AH group 48 hours after surgery (P<0.05), the level of serum creactive protein was significantly higher in the AH group 48 hours after surgery (P<0.05). By comparing the postoperative clinical indicators of the two groups, the group of LH had less intraoperative bleeding, the maximum postoperative hypothermia, and the postoperative hospital stay was short. Conclusion: LH is a minimally invasive surgery because of its mild trauma and little changes in immune function.
