摘 要 目的：研究口腔颌面锥形束CT(CBCT)在微创拔牙术中的应用效果及安全性。方法：选择因需要拔除下颌阻生智齿进行微创拔牙术的患者350例，并分为CBCT组和X线组。CBCT组进行CBCT检查，X线组拍摄曲面体层X线片，两组均采用微创拔牙术进行治疗。观察两组患者的下颌神经损伤发病率、面部肿胀、VAS评分、患者满意度以及邻牙松动、牙根撕裂、断根、张口受限程度以及舌侧骨板骨折等并发症的发生率。结果：CBCT组在术前手术难度高级、牙根与下颌管位置关系重叠中下颌神经损伤的发病率显著低于X线组（P＜0.05）。CBCT组面部肿胀发生率和VAS评分显著低于X线组（P＜0.05），CBCT组患者满意度显著高于X线组（P＜0.05）。 CBCT组和X线组邻牙松动、牙根撕裂、断根、张口受限程度及舌侧骨板骨折的发生率经比较，差异无统计学意义（P＞0.05）。结论：通过CBCT对下颌阻生智齿拔除术做出术前风险评估，同时联合微创拔牙术，能够减少下颌神经损伤的发生，提高患者的满意度，且安全性较高。
Abstract:
Abstract Objective：To study the application and safety of oral and maxillofacial cone beam CT in minimally invasive extraction. Methods：350 patients who underwent minimally invasive extraction on mandibular impacted wisdom teeth were selected, and were divided into CBCT group and Xray group.The CBCT group was examined by CBCT, and the Xray group was photographed with curved body Xray films. Both groups were treated with minimally invasive extraction.The incidence of mandibular nerve injury, facial swelling, VAS score, patient satisfaction, and the incidence of complications such as loosening of the teeth, root tear, root fracture, degree of mouth opening, and lingual bone fracture were observed. Results：In the CBCT group, the incidence of advanced mandibular nerve injury and the position of the mandibular and mandibular canal were significantly lower than that in the Xray group (P<0.05).The facial swelling degree and VAS score in the CBCT group were significantly lower than those in the Xray group (P<0.05). The satisfaction of the CBCT group was significantly higher than that in the Xray group (P<0.05).There was no significant difference in the incidence of adjacent teeth loosening, root tear, root fracture, open mouth limitation and lingual bone fracture in CBCT group and Xray group (P>0.05). Conclusion：Preoperative risk assessment of mandibular impacted wisdom tooth extraction by CBCT, combined with minimally invasive extraction, can reduce the occurrence of mandibular nerve injury, improve patient satisfaction, and high safety.
