摘 要 目的: 探讨血清βD葡聚糖(βDG)、半乳甘露聚糖(GM)、和CandTec抗原检测在诊断侵袭性真菌病的应用价值。方法: 选择收治的疑似侵袭性真菌病住院患者136例作为研究对象,根据相关诊断标准确定是否发生侵袭性真菌病分为感染组82例和未感染组54例,使用Latex凝集法检测血清CandTec抗原,使用ELISA法检测GM抗原,使用酶促反应检测βDG水平,分析三者联合诊断侵袭性真菌病的灵敏度和特异度。结果: 感染组患者βDG阳性检出率为52.44%,GM阳性检出率为69.51%,CandTec阳性检出率为80.49%,三者联合检出率为91.46%,组间比较差异具有统计学意义(P<0.05);三者联合检测时灵敏度、特异度和符合度分别为95.12%,98.78%、92.68%,均明显高于三项指标单独检测值,差异具有统计学意义(P<0.05)。结论: 血清βDG、GM抗原和CandTec抗原联合应用能够提高IFD诊断的敏感度、特异度和符合度,均高于单独选择某种试验,在实际工作中建议三者联合使用,同时可结合其他检测方法和技术进行综合诊断进一步提高诊断的准确率。
Abstract:
AbstractObjective: To investigate the value of serum betaD glucan (betaD Glucan, betaDG), galactomannan (GM), and CTec antigen in the diagnosis of invasive mycosis. Methods :136 hospitalized patients with suspected invasive mycosis admitted to our hospital from August 2017 to July 2018 were divided into two groups according to the relevant diagnostic criteria: 82 cases of infection group and 54 cases of noninfection group. The serum CandTec antigen was detected by Latex agglutination, GM antigen was detected by ELISA, and GM antigen was detected by enzyme. The level of beta DG was detected by stimulating reaction, and the sensitivity and specificity of combined diagnosis of invasive fungal disease were analyzed by the three methods. Results :The positive rate of betaDG was 52.44%, GM was 69.51%, CandTec was 80.49%, and the combined detection rate was 91.46%. There was significant difference between the three groups (P<0.05); the sensitivity, specificity and coincidence were 95.12%, 98.78% and 92.68%, respectively. The difference was statistically significant (P<0.05), which was significantly higher than the three indicators alone. Conclusion :The combination of serum betaDG, GM antigen and CTec antigen can improve the sensitivity, specificity and coincidence of IFD diagnosis, which are higher than that of a single test. It is suggested that the combination of the three antigens should be used in practical work, and the diagnostic accuracy can be further improved by combining other detection methods and techniques.
