摘要:
摘 要 目的:探讨输血质量控制对输血安全性和凝血因子的影响。方法:选取进行输血7230例中100例存在不同问题输血患者的血液标本,采用随机数表法根据入院时间的先后分为对照组和观察组各50例。对照组按常规的输血流程步骤操作,观察组应用输血质量控制措施进行分析,比较两组患者的输血安全隐患发生率、不良事件发生情况及凝血因子指标。结果:对照组出现7例交叉配血,11例写错血型,6例输血前后未及时消毒,4例采错血;观察组出现1例交叉配血,3例写错血型,1例采错血;观察组发生交叉配血、写错血型、输血前后消毒以及采错血的比例均显著低于对照组(P<005)。对照组出现8例血液污染,6例溶血反应,15例严重过敏,6例急性肺损伤;观察组出现5例严重过敏, 观察组发生血液污染、严重过敏和急性肺损伤的比例显著低于对照组(P<005)。两组患者输血前2 h内凝血酶原时间、活化部分凝血活酶时间、凝血酶时间、血小板计数和纤维蛋白原比较差异无统计学意义(P>005),但对照组术后2h内凝血酶原时间、活化部分凝血活酶时间、凝血酶时间均显著长于观察组(P<005),血小板计数和纤维蛋白原水平显著低于观察组(P<005)。结论:输血过程中采取质量控制措施能保障输血的安全性,降低输血安全事故的发生率、不良事件的发生情况和凝血因子水平的变化。
Abstract:
Abstract Objective:To explore the effect of blood transfusion quality control on blood transfusion safety and clotting factorsMethods:Blood samples from 72 patients with transfusions were selectedThe random number table method was divided into control group and observation group according to the time of admissionFor example,the control group was operated according to the routine blood transfusion procedureThe observation group was analyzed by blood transfusion quality control measures,and the incidence of blood transfusion safety hazards,adverse events and coagulation factors were compared between the two groupsResults:In the control group,7 cases were crossmatched,11 cases were mistyped,6 cases were not disinfected in time before and after transfusion,4 cases were misplaced,1 case was crossmatched in the observation group,3 cases were mistyped,and 1 case was misplacedThe proportions of crossmatching,misblood type,disinfection before and after transfusion,and misplaced blood in the observation group were significantly lower than those in the control group (P<005)In the control group,8 cases of blood contamination,5 cases of hemolysis,and 15 cases of severe allergies were observedFive cases of acute lung injury,5 cases of severe allergies in the observation group,the incidence of blood pollution,severe allergies and acute lung injury in the observation group was significantly lower than the control group (P <005);two groups of patients within 2h before transfusion prothrombin time.There was no significant difference in activated partial thromboplastin time,thrombin time,platelet count and fibrinogen (P>005),but prothrombin time,activated partial thromboplastin time and thrombin time in the control group within 2 hours after operationBoth were significantly longer than the observation group (P<005),but the platelet count and fibrinogen levelswere significantly lower 〖LM〗 than the observation group (P<005)Conclusion:quality control measures can ensure the safety of blood transfusion,reduce the incidence of transfusion accidents,adverse events and changes of coagulation factors
