摘 要 目的:探讨超微血流成像在评价颈动脉硬化斑块稳定性中的应用及其对脑卒中的预测价值。 方法:选取常规超声检查诊断为颈部动脉硬化斑块患者72例,其中脑梗塞组40例,非脑梗塞组32例。对两组进行超微血流成像(SMI)及超声造影检查(CEUS),比较两者对颈动脉斑块稳定性评估的一致性及其在预测缺血性脑卒中的价值。结果:CEUS、SMI检查技术对显示硬化斑块内新生血管方面比较差异无统计学意义(P> 0.05)。CEUS、SMI检查技术在颈动脉硬化斑块内新生血管分级方面比较差异无统计学意义(P> 0.05)。以脑梗塞的发生为因变量,以颈动脉不稳定斑块数量及SMI检查方法所获得的斑块内血流信号评分作为自变量,进行Logistic 多因素回归分析发现:颈动脉斑块不稳定(OR=1.94, 95% CI: 1.09~4.37,P= 0.02)、SMI 分值(OR = 3.57, 95%CI: 1.87~8.84, P = 0.007)均为脑梗塞的独立危险因素。脑梗塞组患者颈部不稳定斑块发生率(87.50%)及SMI血流评分(3.14±0.96)分均显著高于非脑梗塞组(42.50%)、(0.85±0.34)分,两者间比较差异有统计学意义(P< 0.05)。结论:SMI对颈动脉斑块内新生血管的评估与CEUS具有良好一致性,可用于评估颈动脉斑块稳定性,其可预估颈动脉硬化斑块患者罹患缺血性脑卒中风险。
Abstract:
Abstract Objective:To investigate the application of superb micro-vascular Imaging in evaluating the stability of carotid atherosclerotic plaques and its predictive value for stroke. Methods:72 cases of cervical atherosclerotic plaques were diagnosed by conventional ultrasound, including 40 patients in the ischemic stroke group and 32 cases of non ischemic stroke group. The ultrahigh blood flow imaging and ultrasound contrast examination were performed in two groups, and the stability of the carotid artery plaque was compared with the ultrasound contrast. The consistency of the assessment and its value in predicting ischemic stroke. Results: There was no significant difference in the neovascularization between CEUS and SMI (P> 0.05). There was no significant difference in the classification of neovascularization in carotid atherosclerotic plaques between CEUS and SMI (P>0.05). In the study of the occurrence of ischemic stroke as a dependent variable, the number of unstable plaque in the carotid artery and the score of blood flow signal in the plaque obtained by SMI examination were used as independent variables. After the multiple regression analysis of Logistic, carotid plaque instability (OR = 1.94, 95%CI: 1.09-4.37, P= 0.02), SMI score (OR = 3.57, 95%CI: 1.87-8.84, P = 0.007) were independent risk factors for ischemic stroke. The incidence of unstable plaque in the neck of the ischemic stroke group (85.0%) and the SMI blood flow score (3.14±0.96) were significantly higher than those in the non ischemic stroke group (53.13%) and (0.85±0.34). There was a significant difference between the two groups (P< 0.05). Conclusion: The evaluation of the neovascularization in carotid artery plaque with SMI is consistent with CEUS, and it can be used to evaluate the stability of carotid artery〖LM〗 plaque. It can predict the risk of ischemic stroke in patients with carotid atherosclerotic plaque.
