摘 要 目的:研究系统性红斑狼疮(SLE)患者亚临床动脉粥样硬化的发生情况以及影响因素。方法:选择SLE患者58例和性别、年龄匹配的健康对照28例,收集患者年龄、性别、病程、SLE疾病活动指数(SLEDAI)等临床资料,测定患者血常规、尿常规、血脂、肝功、肾功、血沉、补体、免疫球蛋白等指标,采用颈动脉超声检查内膜中层厚度、颈动脉狭窄程度,测定脉搏波传导速度(PWV)。比较分析病例组与对照组亚临床动脉粥样硬化的发生情况,分析与亚临床动脉粥样硬化发生相关的危险因素。结果:病例组颈动脉IMT显著高于对照组[(0.56±0.17) mm与(0.39±0.03) mm,t=5.236 ,P<0.01]。年龄小于40岁的SLE患者的PWV明显高于健康对照组(1339.8 cm/s与1044.6 cm/s,t=2.589,P=0.019)。相关分析显示,SLEDAI(r=0.335,P=0.015)、强的松累积剂量(r=0.742,P<0.001)和病程(r=0.741,P<0.001)是颈动脉IMT的相关危险因素。结论:SLE患者的亚临床动脉粥样硬化风险明显增加,而且疾病活动指数、强的松累积剂量和病程是颈动脉IMT增厚的相关危险因素。
Abstract:
Abstract Objective: To study the occurrence of subclinical atherosclerosis and its influencing factors in patients with systemic lupus erythematosus (SLE). Methods: A total of 58 SLE patients and 28 healthy controls were selected. Age, sex, course of disease, SLE disease activity index (SLEDAI) and other clinical data were collected. Blood routine, urine routine, blood lipids, liver function, renal function, ESR, complement, immunoglobulin, and other indicators were measured. Carotid artery ultrasound were used to measure intima-media thickness (IMT) and carotid stenosis. Pulse wave velocity (PWV) was also measured. The incidence of subclinical atherosclerosis in case group and control group was compared and the risk factors associated with subclinical atherosclerosis was analyzed. Results: IMT was significantly higher in the case group than in the control group (P<0.01). The PWV of SLE patients who are younger than 40 years were significantly higher than that of healthy controls (1339.8 vs. 1044.6; P=0.018). Correlation analysis showed that SLEDAI, prednisone cumulative dose and course of disease were related to carotid IMT. Conclusion: The risk of subclinical atherosclerosis in patients with SLE is significantly increased, andSLEDAI, cumulative dose of prednisone and disease duration are related to carotid IMT.
