目的:研究了甲状旁腺激素(1-34) \[PTH(1-34)\]联合鲑鱼降钙素针剂对绝经后骨质疏松(PMO)压缩性骨折患者骨密度、骨代谢的影响。方法:选择因PMO压缩性骨折进行治疗的患者120例,随机分为联合组和对照组,各60例。对照组在常规治疗的基础上使用鲑鱼降钙素注射液,联合组在对照组的基础上使用PTH(1-34),两组均治疗6个月。观察两组患者治疗前、治疗3个月和6个月时的骨密度、骨钙素(BGP)、β-胶原降解产物(β-CrossLaps)、骨碱性磷酸酶(BALP)、I型胶原羧基末端交联肽(CTX-1)、VAS和ODI评分的变化。结果:与联合组相比,治疗6个月后,对照组腰椎和大转子的骨密度显著较低(P<0.05),股骨颈的骨密度没有统计学差异(P>0.05);对照组BGP和BALP的变化显著较低(P<0.05),β-CrossLaps和CTX-1的变化显著高于联合组(P<0.05);随着治疗时间的增长,两组患者的VAS和ODI评分显著降低(P<0.05),治疗3个月和6个月时,与联合组相比,对照组的VAS和ODI评分显著较高(P<0.05)。结论:PTH(1-34)和鲑鱼降钙素针剂联合使用能够显著增加患者骨密度,提高骨代谢中BGP和BALP含量,降低β-CrossLaps和CTX-1含量,改善患者的疼痛程度及生活质量。
Abstract:
Objective:The effect of parathyroid hormone (1-34) combined with salmon calcitonin injection on bone mineral density and bone metabolism in postmenopausal osteoporotic compression fractures were studied in this paper. Methods:120 patients who had compression fractures of PMO were randomly divided into a combination group and a control group, 60 cases each. In the control group, salmon calcitonin injection was used on the basis of conventional treatment, and PTH (1-34) was used in the combined group on the basis of the control group. Both groups were treated for 6 months. Changes in bone mineral density, BGP, β-CrossLaps, BALP, CTX-1, VAS, and ODI scores were observed before and at 3 and 6 months of treatment in both groups.Results:Compared with the combination group, after 6 months of treatment, the bone density of the lumbar spine and greater trochanter in the control group was significantly lower (P<0.05), and the bone mineral density of the femoral neck was not statistically different (P>0.05). The changes of BGP and BALP in the control group were significantly lower (P<0.05), and the changes of β-CrossLaps and CTX-1 were significantly higher than that of the combined group (P<0.05). Compared with the combination group, the VAS and ODI scores of the two groups decreased significantly with the increase of treatment time (P<0.05). At 3 months and 6 months of treatment, compared with the combined group, the VAS of the control group and ODI score was significantly higher (P<0.05). Conclusion:The combination of PTH (1-34) and salmon calcitonin injection can significantly increase bone mineral density, increase BGP and BALP content in bone metabolism, reduce β-CrossLaps and CTX-1 levels, and improve the patient's pain and quality of life, which could be widely used in the clinical application.
